Job summary

Haven Home Care Inc. is expanding its proven home care model into Eastern Ohio, seeking an LPN for a per-visit supervisory role alongside another job. You will complete supervisory visits, open visits, and skills/written tests, with mileage reimbursed and a company-provided iPad.

No guaranteed weekly hours and no full-time salary expectations. The role covers Ashtabula, Columbiana, Mahoning, and Trumbull counties, with initial client growth as the territory builds.

Qualifications

• Must be at least 18 years old.
• Ohio LPN license in good standing.
• Valid driver's license and reliable transportation.
• Current CPR certification.
• Successful completion of criminal background check, skills test, and written nursing test.

Responsibilities

• Complete open visits and supervisory visits every 60 days.
• Collect and document records per agency standards and regulatory requirements.
• Conduct pre-employment testing for Home Health Aide candidates.
• Support scheduling, documentation, and compliance activities as needed.
