Haven Home Care Inc. is seeking an LPN to join as a per-visit supervisory role in Eastern Ohio. This position sits alongside another job, not as a replacement, and pays per completed visit or task rather than hourly wages.

Initial hours will be minimal as we build the client base in Ashtabula, Columbiana, Mahoning, and Trumbull counties. Travel to client homes is required; mileage is reimbursed.

Qualifications

• Must be at least 18 years old.
• Graduation from an accredited School of Practical Nursing.
• Current Ohio LPN license in good standing.
• Valid driver's license, reliable transportation, and current auto insurance that covers driving for work.
• Current CPR certification.
• Successful completion of criminal background check, skills test, and written nursing test.
• Residence in Trumbull or Mahoning County preferred.

Responsabilités

• Documentation: Prepare accurate and thorough documentation according to agency standards and regulatory requirements.
• Supervisory and Open Visits: Conduct regular supervisory visits every 60 days to check on Home Health Aides and complete open visits for new clients.
• Pre-Employment Testing: Conduct skills tests and written tests for Home Health Aide candidates.
