Job summary

Vivent Health is seeking a Part-time Case Manager to join our interdisciplinary healthcare team in Chicago. You will provide comprehensive case management, addressing medical, social, and emotional needs while collaborating with physicians, nurses, and counselors to ensure holistic care.

You will document activities, assess client eligibility, and connect clients with community resources and insurance coverage.

Qualifications

• Bachelor’s degree in Social Work, Psychology, or related field required.
• Previous case management experience preferred.
• HIV experience in healthcare or social services setting preferred.

Responsibilities

• Provide comprehensive case management for clients across medical, social, and emotional needs.
• Collaborate with healthcare professionals to coordinate access to services.
• Maintain accurate documentation in compliance with standards.
• Deliver crisis intervention and client advocacy when needed.
• Support teams across programs including prevention and telehealth as required.
