Job summary

AIDS Foundation of Chicago is seeking a Medical Case Manager to provide client‑centered, confidential services that link clients to medical, dental, mental health, substance use and supportive care, including transportation, housing and emergency assistance. The goal is viral suppression by removing barriers to care and supporting treatment adherence.

This full‑time, on‑site role in Chicago requires an associate degree in human services and four years of case management experience; bilingual

Qualifications

• Associate degree in any human services field.
• Four years or more of Case management experience.
• Bachelor’s degree in any human services field (preferred).
• Bachelor’s degree in any non-human services field with at least 2 years of case management experience (preferred).
• Bilingual in reading, writing and speaking Spanish (preferred).

Responsibilities

• Conduct face-to-face assessments and annual reassessments.
• Create care plans that match identified client needs.
• Ensure clients are eligible for services before delivery.
• Provide care linkage and re-engagement support, including older adults.
• Conduct field visits to RW and HOPWA clients.
• Refer clients to services highlighted in the care plan.
• Document encounters in the client database accurately and timely.
• Develop relationships with social service, health, and governmental agencies.
• Participate in supervision, team meetings, and trainings.
