Job summary

The AIDS Foundation Chicago seeks a Medical Case Manager in Chicago, IL to link clients with medical, dental, mental health and other services and to help ensure viral suppression by reducing barriers to care. The role involves treatment adherence counseling and enrolling eligible clients in the 340B program.

Qualifications

• Associate degree in any human services field.
• Four years or more of case management experience.
• Bachelor’s degree in any human services field.
• Bachelor’s degree in any non-human services field with at least 2 years of case management experience.
• Bilingual in reading, writing and speaking Spanish.

Responsibilities

• Assessments & service plans: face-to-face assessments and annual reassessments; create care plans; ensure eligibility before service.
• Client service: linkage and re-engagement for clients including older adults; field visits for RW and HOPWA clients.
• Outreach and referrals: refer clients to services in care plans; screen for income supports and benefits; assist with emergency financial assistance.
• Documentation & coordination: complete payment requests; document encounters in the client database; develop relationships with social service, health and government agencies.
• Meetings & training: participate in regular supervision, team meetings and in-house trainings.
