Job summary
Positive Impact Health Centers is seeking a Medical Case Manager to provide comprehensive case management for adults living with HIV, working within an interdisciplinary team including medical and behavioral health professionals. Qualifications
- Strong social work, nursing, or case management skills.
- Excellent interpersonal and cross-cultural communication abilities.
- Spanish language fluency is highly preferred; English proficiency required.
Responsibilities
- Provide medical case management services to eligible clinic patients.
- Develop comprehensive ISP and monitor its progress.
- Coordinate referrals for medical and non-medical services.
- Maintain HIPAA-compliant documentation in EMR.
- Collaborate with an interdisciplinary team to meet patient needs.
