You will assess, plan, advocate, and connect clients with community resources, ensuring timely access to services; maintain documentation; participate in a multidisciplinary team to improve outcomes in HIV care and prevention.

Qualifications

• Bachelor’s degree required in a related field or equivalent.
• Experience in case management preferred in healthcare or social services.
• Valid Government Issued Driver’s License required.

Responsabilités

• Oversee comprehensive case management for clients within the healthcare team.
• Collaborate with providers to coordinate access to medical services.
• Perform eligibility assessments and document supporting proof for grant-funded services.
• Assist clients in accessing community resources and insurance coverage.
• Advocate for clients within the healthcare system and provide crisis intervention as needed.
