PrimeCare Health, Inc. is seeking a full-time Ryan White Program Case Manager in Chicago. The role focuses on comprehensive medical case management to promote viral suppression and coordinated access to HIV-related services.

Responsibilities include intake, care planning, and collaboration with care teams and insurers. Strong communication, attention to detail, and flexibility for travel between sites are essential.

Qualifications

• Six (6) years of work experience OR related education at required level.
• HIV/AIDS experience preferred.
• Bilingual English/Spanish preferred but not required.

Responsabilités

• Oversee and manage an assigned caseload of patients living with HIV/AIDS through Medical Case Management.
• Perform intake and ongoing assessments of patient needs, including medical status, insurance, finances, adherence, and psychosocial needs.
• Develop comprehensive care plans and monitor progress toward goals.
• Maintain patient files per PrimeCare Community Health and AFC standards.
• Collaborate with medical teams, hospitals, and insurers to coordinate integrated care.
• Engage in outreach, trainings, and development projects as needed.
