Vivent Health in Chicago is seeking a part-time Case Manager (24 hours/week) to provide comprehensive, compassionate management of clients' medical, social, and emotional needs. You will collaborate with healthcare professionals to ensure access to care and coordinated services.

The role emphasizes crisis intervention, documentation, and advocacy within an equity-driven, inclusive workplace. A Bachelor's degree in Social Work, Psychology, or related field is expected, with prior case management

Qualifications

• Bachelor’s degree in Social Work, Psychology, or a related field required.
• Experience in case management preferred and healthcare/social services exposure.
• Familiarity with coordinating patient care plans and accessing community resources.
• Ability to learn multiple databases and software systems.
• Strong interpersonal and communication skills; commitment to confidentiality.

Responsibilities

• Collaborates with HIV testing sites, prevention team, clinical staff and local providers to initiate access to care.
• Conducts thorough assessments to develop individualized case management plans.
• Coordinate and facilitate access to medical services for holistic patient care.
• Perform eligibility assessments and collect documentation for grant-funded services.
• Advocate for clients within the healthcare system and provide crisis intervention when needed.
