Job summary

Boston Health Care for the Homeless Program is seeking a Medical Case Manager to join our multidisciplinary HIV care team. You will assess barriers to care, connect patients with housing, benefits, and behavioral health resources, and coordinate care with medical and community partners.

Qualifications

• Associate or bachelor’s degree in a related field and/ or equivalent two years relevant human services experience.
• Experience working with people with substance use disorder is required; homelessness experience is a plus.
• Commitment to harm reduction and trauma-informed approaches; street outreach experience is a plus.
• Commitment to equitable and culturally appropriate care for diverse populations; includes communities of color, LGBTQIA, non-English speakers.
• Bi-lingual fluency in Spanish is preferred.

Responsibilities

• Assess new HIV patients for financial, psycho-social, housing, and other needs; develop comprehensive service care plan; assess Ryan White eligibility; attend trainings.
• Work with patients to reduce barriers to full engagement in HIV care; arrange transportation; accompany to appointments; track missed appointments; document care in medical records.
• Identify patients who will benefit from outreach visits to shelters, outside agencies, jail, or home; participate in care team meetings to coordinate care.
• Conduct intensive outreach visits on streets or at shelters to connect with patients newly diagnosed with HIV or not yet in care.
