Qualifications

Responsibilities

• Assist individuals with goal completion per the individualized support plan.
• Help with Activities of Daily Living (ADLs) such as bathing, grooming, toileting, and feeding.
• Safely transport individuals in company or personal vehicles.
• Accompany individuals to community activities and events.
• Complete required documentation in the electronic health record (EHR) in a timely manner.
• Engage with individuals in their home and community while upholding rights and privacy.
