Job summary

Responsibilities

Assist individuals with goal completion per the individualized support plan.
Provide ADL support (bathing, grooming, toileting, feeding, meal prep) as needed.
Safely transport individuals in company or personal vehicles.
Accompany individuals to community integration activities.
Complete required documentation in the electronic health record (EHR) timely.
Uphold rights and maintain HIPAA confidentiality.
Maintain professional boundaries and ethical relationships.
Mandated reporter: report any abuse, neglect, or exploitation immediately.
