AdventHealth is seeking a Part-time Care Management professional to support discharge planning and post-acute coordination in Parker, Colorado. The role involves counseling patients and families, assessing needs, and coordinating with multiple teams to ensure timely care transitions.

Ideal candidates will have a Master's degree in a relevant field, 2+ years in care management or social work, and familiarity with discharge planning processes.

Formación

• Master's degree required.
• 2+ years in care management preferred; 2+ social work required.
• ACM/ CCM certifications preferred.

Responsabilidades

• Provide grief counseling, disease adjustment support, and discharge planning for inpatients and ED patients.
• Assess discharge needs and plan post-acute care, coordinating with families and care teams.
• Document care plans, MDRs, and barrier to progression; advocate for patient and family needs.
