Job summary

AdventHealth Apopka in Florida is seeking a Care Manager to provide discharge planning and patient advocacy within a high-volume hospital. The role requires a Master’s degree and experience in care management and social work, with responsibilities spanning inpatient, observation, and ED settings.

You will collaborate with a multidisciplinary team to coordinate post‑acute care, support families, and ensure timely transitions while working a day shift in Apopka.

Qualifications

• Master's degree required.
• 2+ care management experience preferred.
• 2+ social work experience required.

Responsibilities

• Provides grief counseling, crisis intervention, goals of care planning, and de-escalation as appropriate.
• Assesses discharge planning needs across inpatient, observation and/or emergency departments to avoid delays.
• Reviews medical records and factors into the transition of care plan for discharge.
• Develops discharge plans with contingency planning to ensure timely care coordination.
• Participates in rounds and communicates with post-acute care services and payors for authorization and coordination.
