Job summary

AdventHealth is seeking a Registered Nurse with strong discharge planning and care coordination skills. You will lead multidisciplinary rounds, assess patient needs, and educate families on care options to ensure smooth transitions from hospital to post-acute settings.

The role requires 2+ years of medical/hospital nursing experience, RN license, and familiarity with CMS discharge planning guidelines. A hybrid of clinical and coordination duties is expected in a Denver-based facility.

Qualifications

• Leadership, problem-solving and communication skills are required.
• Experience in hospital nursing is required (2+ years).
• Knowledge of discharge planning processes and post-acute care is preferred.

Responsibilities

• Participates in multi-disciplinary rounds to review patient status and discharge plans.
• Educates patients and families about illness impacts and available resources.
• Assess readmitted patients' reasons and coordinate care transitions.
• Organizes and leads discharge planning conferences with the care team.
• Documents discharge plans, barriers, and needs in medical records.
• Communicates with Payors for post-acute care authorization as needed.
• Ensure timely discharge planning and coordination across inpatient, observation, and ED settings.
