Job summary

AdventHealth Port Charlotte is seeking an experienced Care Manager to coordinate discharge planning and post-acute care for inpatients, observation and ED patients. You will assess needs, collaborate with families, payors and the multidisciplinary team to ensure timely transitions and patient-centered support.

The role emphasizes compassionate care, community resources navigation and adherence to quality standards. A Florida license in social work or case management is preferred.

Qualifications

• 2+ care management experience (Preferred)
• 2+ social work (Required)
• Additional Licensure or certification requirements may apply depending on the unit/state. Refer to credential grid for details.

Responsibilities

• Provide grief counseling, disease adjustment support, crisis intervention, goals of care planning and de-escalation services for patients as appropriate.
• Assess patients and families holistically for discharge planning needs in inpatient, observation and/or emergency departments.
• Review medical records and incorporate clinical, social and financial factors into the transition of care plan.
• Develop discharge plans with contingency plans for timely care coordination and post-acute services.
• Communicate with Payors for authorization for post-acute care as needed and participate in multidisciplinary rounds.
• Escalate delays to leadership when discharge planning is impeded and advocate for patient and family empowerment.
