Job summary

AdventHealth Parker is seeking an experienced Registered Nurse Case Manager to coordinate discharge planning and ensure timely transitions of care for hospitalized patients.

In this role, you will lead multidisciplinary rounds, evaluate patient needs, liaise with payors for post-acute authorization, and develop comprehensive discharge plans across inpatient, observation and ED settings. Part-time schedule, day shifts.

Qualifications

• RN license required.
• Associates degree in nursing required; Bachelor's preferred.
• 2+ years of medical/hospital nursing experience.
• Care management/Utilization Management experience preferred.

Responsibilities

• Actively participates in multidisciplinary rounds to review patient status and discharge plans.
• Educates patients and families on illness impacts and arranges community resources.
• Assess readmission reasons for patients and families.
• Facilitates patient/family care conferences with the team.
• Documents discharge planning evaluations and barriers to progression.
• Communicates post-acute care needs to payors for authorization.
• Assesses inpatient, observation, and ED patients for discharge needs to avoid delays.
• Reviews medical records and integrates clinical, social, and financial factors into discharge plans.
• Develops discharge plans with contingency measures for timely care coordination.
• Uses standard workflows and technology to ensure complete medical records and accurate discharge reconciliation.
• Performs other duties as assigned.
