AdventHealth in Florida is seeking a Care Manager to oversee discharge planning, post‑acute care coordination, and patient advocacy within inpatient and ED settings. You will review records, coordinate with families, and develop contingency plans to ensure timely progression of care.

The role emphasizes collaboration with multidisciplinary teams, data‑driven decision making, and effective communication with payors and community resources to support patients’ needs across the care continuum.

Responsabilités

• Provide discharge planning, disease-adjustment support, crisis intervention and care coordination.
• Assess patients and families for discharge needs across inpatient, observation and ED settings.
• Review medical records and integrate clinical, social, and financial factors into care plans.
• Develop discharge plans and coordinate post-acute services and community resources.
• Communicate with post-acute providers to ensure continuity of care and accurate records.
• Participate in multidisciplinary rounds to optimize patient outcomes and resource use.
• Document evaluations, plans, barriers, and patient advocacy during hospitalization.
• Coordinate with payors for post-acute care authorization when needed.
• Identify reasons for readmission and implement prevention strategies.
