Job summary

AdventHealth Littleton is seeking a part-time Discharge Planner RN to join our multidisciplinary care team. The role focuses on coordinating discharge planning, reviewing patient needs, and coordinating post-acute services to ensure timely transitions.

You will participate in rounds, educate patients and families, document plans, and advocate for patients’ social and financial needs while maintaining CMS and hospital policy compliance.

Qualifications

• Leadership and analytical abilities required.
• Minimum 2+ years medical/hospital nursing experience.
• RN license, BLS and CCM/ACM preferred.

Responsibilities

• Participates in multidisciplinary rounds to review patient status, discharge plans and progression.
• Educates patients and families about illness impacts and mobilizes resources for identified needs.
• Assesses readmitted patients and reasons for readmission.
• Organizes and facilitates patient/family care conferences with the team.
• Documents discharge planning evaluation, barriers, and post-acute care authorizations.
• Develops discharge plans with contingency plans for post-acute services.
