AdventHealth in Littleton, CO is seeking a part-time Registered Nurse to join a collaborative care team. The role covers 24 hours per week across three 8-hour shifts, with every other Sunday required.

You will participate in multidisciplinary rounds, coordinate discharge planning, and communicate plans for post-acute care to patients and families. Ideal candidates bring an Associate Degree in Nursing and an active RN license, with 2+ years of hospital nursing experience and strong skills in

Qualifications

• Active RN license is required.
• Minimum 2 years of medical/hospital nursing experience.
• Associate degree in nursing required; bachelor's preferred.
• Strong discharge planning and patient education skills.

Responsabilités

• Participate in multi-disciplinary rounds to review patient status and discharge plans.
• Coordinate discharge planning with care teams and payors for post-acute care.
• Educate patients and families on care plans and psychosocial/economic aspects of illness.
• Document discharge plans, transitions of care, and barriers to progression in the medical record.
• Provide patient-centered care and communicate effectively with diverse populations.
