AdventHealth is seeking an RN Case Manager to join a weekend-friendly, part-time schedule in Denver, CO. The role emphasizes discharge planning, patient education, and resources coordination to support timely transitions of care.

You will work with a multi-disciplinary team, reviewing patient status, planning dispositions, and engaging families to ensure high-quality, person-centered care while navigating post-acute services and payer requirements.

Qualifications

• RN required and active licensure in the appropriate state.
• Associates degree in Nursing required; bachelor's preferred.
• Experience in care management/utilization management preferred.

Responsibilities

• Participate in rounds to review patient care plans and discharge needs.
• Educate patients and families about illness, resources and care options.
• Assess readmissions for reasons and plan improvements.
• Facilitate care conferences with multidisciplinary team.
• Document discharge plans and communicate with payors as needed.
• Coordinate post-acute care and community resources for discharge.
• Ensure continuity of care information and accurate medical records.
