Intermountain Health is seeking a Palliative Care RN to join the team at St Vincent Regional Hospital in Billings, MT. You will coordinate care, educate families, and collaborate with physicians, social workers, and hospice teams to improve quality of life for patients with serious illness.

Qualifications

• Current RN license in the state of practice; compact licensure transfer within 60 days.
• BSN from an accredited institution; education verified.
• BLS certification for Healthcare Providers.
• RNs with less than 12 months of experience must obtain a BSN within four years of hire.
• Experience in palliative care or related fields is preferred.
• Strong clinical assessment and documentation skills.

Responsibilities

• Coordinate care and serve as navigator for palliative care patients and families.
• Educate patients and families on diagnosis, treatment options, and resources.
• Assess symptoms and support symptom management plans with providers.
• Triage calls, questions, and care concerns from patients and families.
• Coordinate services across primary care, hospice, home health, and community resources.
• Support advance care planning conversations and documentation.
• Provide ongoing follow-up and care coordination throughout the patient journey.
