Job summary

Intermountain Health St Vincent Regional Hospital in Billings, Montana is seeking a Palliative Care RN to coordinate care, educate patients and families, and support symptom management across the care continuum.

The role emphasizes holistic, patient- and family-centered care with a Monday–Friday schedule (8:00 AM–5:00 PM), no routine nights/weekends/holidays, and a comprehensive benefits package.

Qualifications

• Current RN license in the state of practice.
• BSN from an accredited institution; education will be verified.
• BLS certification for Healthcare Providers.
• RNs with <12 months of experience must obtain a BSN within four years of hire.

Responsibilities

• Coordinate care and serve as a navigator for palliative care patients and their families.
• Provide patient and family education regarding diagnosis, treatment options, and available resources.
• Assess symptoms and collaborate with providers to support symptom management plans.
• Triage patient and family calls, questions, and care concerns.
• Coordinate services across primary care, specialty care, hospice, home health, and community resources.
• Support advance care planning conversations and documentation.
• Provide ongoing follow-up and care coordination throughout the patient's healthcare journey.
