Qualifications

• Current Registered Nurse (RN) license in the state of practice; compact licenses must transfer within 60 days.
• BSN degree from an accredited institution; education will be verified.
• BLS certification for Healthcare Providers.
• RNs with less than 12 months of experience must obtain a BSN within four years of hire.

Responsibilities

• Coordinate care and serve as navigator for palliative care patients and families.
• Educate patients and families on diagnosis, treatment options, and available resources.
• Assess symptoms and support symptom management plans with providers.
• Triage calls, questions, and care concerns from patients and families.
• Coordinate services across primary care, specialty care, hospice, home health, and community resources.
• Support advance care planning discussions and documentation.
• Provide ongoing follow-up and care coordination throughout the patient journey.
