Job summary

Intermountain Health St Vincent Regional Hospital in Billings, Montana, is seeking a Palliative Care RN to provide compassionate, patient- and family-centered care for seriously ill patients, coordinating across teams.

Qualifications

Current RN license in state of practice; compact license transfers within 60 days.
BSN from accredited institution; education will be verified.
BLS certification for Healthcare Providers.
RNs with <12 months experience must obtain BSN within four years of hire.

Responsibilities

Coordinate care and serve as a navigator for palliative care patients and their families.
Provide patient and family education regarding diagnosis, treatment options, and available resources.
Assess symptoms and collaborate with providers to support symptom management plans.
Triage patient and family calls, questions, and care concerns.
Coordinate services across primary care, specialty care, hospice, home health, and community resources.
Support advance care planning conversations and documentation.
Provide ongoing follow-up and care coordination throughout the patient's healthcare journey.
