Qualifications

• Current RN license in the state of practice (or compact license with transfer within 60 days).
• BSN from an accredited institution. Education will be verified.
• BLS certification for Healthcare Providers.
• RNs with <12 months experience must obtain BSN within 4 years of hire.

Responsibilities

• Coordinate care and navigate palliative care patients and families.
• Educate patients and families on diagnosis, treatment options, and resources.
• Assess symptoms and collaborate with providers on symptom management plans.
• Triage patient and family calls, questions, and care concerns.
• Coordinate services across primary care, specialty care, hospice, home health, and community resources.
• Support advance care planning conversations and documentation.
• Provide ongoing follow-up and care coordination throughout the journey.
