Job summary

Compassus is seeking a Nurse Practitioner - PPV to provide palliative and hospice care across patient care locations. The NP will assess, educate, manage symptoms, and coordinate with PCPs, specialists, and the Interdisciplinary Team.

The role requires a Master’s in Nursing, NP license, and licensure in the state of employment. Experience in hospice/palliative settings is preferred. Willingness to travel and flex schedule as needed is essential.

• Strong written and verbal communication.
• Willingness to work flexible hours and travel.

Responsibilities

• Performs routine and emergency assessments for Hospice Care and Palliative Care.
• Collaborates with Medical Director, Attending Physician, and IDT to coordinate care.
• Educates patients and caregivers on disease process, symptom management, and goals of care.
• Documents visits, care plans, and treatments accurately and promptly.
• Performs other duties as assigned.
