Job summary

Compassus seeks a Palliative Nurse Practitioner to deliver high-quality palliative and hospice care, partnering with the IDT and Medical Director to manage symptoms and care plans. You will assess patients in home or facility settings, educate families, and ensure documentation compliance with CMS guidelines.

The role requires an NP license, strong communication skills, and ability to adapt schedule to patient needs, including after-hours visits as needed.

Qualifications

• Active NP license required in the employment state.
• Board certification in Hospice & Palliative Care preferred but not mandatory.
• Experience in palliative or hospice settings is beneficial.

Responsibilities

• Perform routine and emergency palliative/hospice assessments.
• Collaborate with Medical Director, IDT and PCP to coordinate care.
• Educate patients and families on disease, prognosis, and treatment options.
• Document visits and maintain accurate patient records within CMS guidelines.
