Job summary

Compassus in Wisconsin seeks a Nurse Practitioner - PPV to join our hospice and palliative care team. You will provide routine and emergency assessments, partner with the Medical Director, Attending Physician, DCS and IDT to coordinate patient care, and help patients and families maintain comfort and dignity across care settings.

The role requires a Master’s degree in Nursing, NP licensure in the state, and board certification in Hospice and Palliative Care preferred.

Qualifications

• Master’s degree in Nursing required.
• Active CNS or NP license in state of employment.
• NP certification (Adult, Family, Acute, or Geriatric) preferred.

Responsibilities

• Perform routine and emergency assessments for hospice/palliative care patients.
• Collaborate with Medical Director, Attending Physician, DCS and IDT.
• Provide symptom management and education to patients and families.
• Complete documentation for visits within 24 hours.
