Job summary

Compassus is seeking a Palliative Care Nurse Practitioner to deliver high-quality palliative care across multiple locations. The role emphasizes compassionate, patient- and family-centered care, with collaboration across PCPs and specialists.

Expect to manage assessments, education, and symptom control while adhering to practice standards. The NP will support care coordination within the IDT, participate in regular meetings, and ensure timely documentation.

Qualifications

• Master’s degree in Nursing required.
• Nurse Practitioner license active in state of employment.
• Minimum five years nursing experience; oncology, hospice, geriatrics, or ICU preferred.

Responsibilities

• Perform routine and urgent assessments; manage a patient census and care plans.
• Provide palliative care education, symptom management, and family support.
• Coordinate with Medical Director and IDT; participate in monthly meetings.
