Job summary

Compassus is seeking a Neuro NP (Nurse Practitioner) to deliver palliative and hospice care across patient care locations. You will assess, educate, and manage symptoms in collaboration with PCPs and specialists, while following Compassus values and 6 Pillars of Success.

The role requires a Master’s in Nursing and an NP license; hospice/palliative care certification is preferred. Flex scheduling and after-hours visits may be needed to meet patient needs and maintain comfort and quality of life.

Qualifications

• Master’s degree in Nursing required.
• Active and unencumbered Nurse Practitioner license in state of employment required.
• Board Certification in Hospice and Palliative Care is preferred, but not required.

Responsibilities

• Cooperate with Medical Director, Attending Physician, DCS and IDT to coordinate patient care.
• Perform routine and emergency assessments in hospice or palliative care settings.
• Provide patient and family education on disease processes, symptom management, and care options.
• Record and maintain accurate patient records and documents to support care and reimbursement.
• Assist in development of clinical practice guidelines and standards for quality care.
• Ensure compliance with applicable laws and collaborative agreements.
