Job summary

Compassus in Fort Worth, TX seeks a Palliative Nurse Practitioner to provide comprehensive palliative and hospice care across patient care settings, including home and facility-based visits. You will work closely with the PCP, Medical Director, and interdisciplinary team to assess, educate, and manage symptoms for patients and families.

The role requires a Master’s in Nursing, active NP license (TX), and strong collaboration skills.

Qualifications

• Master’s degree in nursing is required.
• Active Nurse Practitioner license in the state of employment is required.
• Board Certification in Hospice and Palliative Care is preferred but not required.

Responsibilities

• Provides assessments, disease and treatment education, and symptom management.
• Collaborates with Medical Director and IDT to coordinate care.
• Orders medications and treatments within scope of professional practice.
• Maintains accurate patient records and complies with laws and regulations.
