Job summary

The PACE Place seeks an Intake Coordinator to support enrollment into the PACE program by conducting assessments, determining eligibility, and coordinating intake activities across settings.

This role liaises with participants, families, and the IDT, promoting PACE services and ensuring compliance with regulations and timelines. Strong communication and data management skills are essential.

Qualifications

• Understanding of physical, psychosocial, behavioral, and family needs of older adults.
• Knowledge of community health, social service systems, aging networks, and funding sources (e.g., Medicaid, Medicare).
• Familiarity with PACE philosophy, eligibility criteria, and denial processes.
• Effective verbal and written communication, presentation, and organizational skills.
• Ability to work independently, manage time, and coordinate multiple activities.
• Proficiency in Microsoft Word, Excel, PowerPoint, and Outlook.
• Skill in building collaborative relationships with participants, families, and agencies.

Responsibilities

• Conduct intake assessments in homes, hospitals, and facilities.
• Educate prospective participants and caregivers on PACE eligibility and services.
• Input and track referral and intake data using EHR and tracking systems.
• Facilitate Medicaid enrollment documentation submission by deadlines.
• Participate in IDT meetings to present intake findings for enrollment.
• Maintain accurate written records of assessments and activities.
• Collaborate with marketing and enrollment teams for community outreach.
• Build and maintain relationships with community referral sources.
• Prepare data for state and federal reporting as required.
• Evaluate and improve intake and assessment procedures.
• Ensure privacy, infection control, OSHA safety, and quality initiatives.
