Job summary

Midland Care Connection is seeking a PACE Intake Coordinator to guide prospective participants and families through enrollment, gather documentation, and coordinate assessments with the interdisciplinary team.

You will help determine eligibility and ensure a smooth, patient-centered enrollment experience across our PACE program, while safeguarding confidentiality and collaborating with care partners.

Qualifications

• High school diploma or equivalent and relevant healthcare experience.
• 1–2 years in healthcare, social services, customer service, admissions/intake, or related administrative role.
• Strong verbal and written communication skills.
• Excellent organizational skills and attention to detail.

Responsibilities

• Guide potential participants and families through the intake process.
• Gather necessary information and documentation for enrollment.
• Coordinate assessments and appointments with the interdisciplinary team.
• Determine eligibility and readiness for enrollment in PACE.
• Ensure smooth enrollment and a positive experience for participants and families.
