Job summary

CenterLight seeks an Intake Navigator to educate prospective participants through onboarding to enrollment in the PACE program. You will collaborate with referral sources, account managers, and clinical intake to ensure timely processing of enrollments and compliance with Medicaid, Medicare and PACE regulations.

Responsibilities include handling calls, data entry, and scheduling assessments while meeting productivity targets and maintaining quality and satisfaction for participants and referral

Qualifications

• High School Diploma required; Associate degree preferred.
• Minimum 2 years work experience in healthcare; MLTC/MAP/PACE preferred.
• Knowledge of Medicaid, Medicare and PACE regulations.
• Proficiency with CRM platforms; Care Compass strongly preferred.
• Proficiency in Microsoft Excel, Word, Outlook and Teams.

Responsibilities

• Educate prospective participants about the PACE enrollment process.
• Handle inbound/outbound calls, data entry, emails; schedule assessments and mail information.
• Meet departmental productivity goals for company growth.
• Become an expert on CenterLight PACE eligibility requirements, services and benefits.
• Verify plan and entitlement status using LDSS/HRA/CMS resources.
