Qualifications

• CPR (BLS or ACLS) required.
• Valid California Driver’s License is required.
• Minimum 2 years in healthcare, senior services, or enrollment coordination.
• 1 year working with geriatric or frail populations.

Responsibilities

• Educate prospective participants about the PACE model and enrollment eligibility.
• Coordinate screenings, assessments, and enrollment documentation.
• Schedule home visits, center tours, and team evaluations.
• Maintain HIPAA compliant records and timely documentation.
• Collaborate across marketing, outreach, clinical, and leadership teams.
