Job summary

BoldAge PACE is seeking an Enrollment Specialist for a hybrid role to guide prospective participants and their families through the enrollment process, converting qualified referrals into program enrollments. You will be the primary contact, building trusted relationships while explaining benefits and requirements of the PACE program.

• Strong interpersonal skills and ability to build trusted relationships with prospects and caregivers.
• Experience guiding enrollment conversations and translating needs into program enrollment.
• Proficiency with CRM systems and accurate documentation of interactions.

Responsibilities

• Guide prospects through enrollment from intake to completion, aligning needs with PACE services.
• Lead enrollment activities with internal partners to ensure readiness and completion.
• Document all prospect interactions in CRM and EMR systems accurately.
