Job summary

North East Medical Services (NEMS) is seeking an Enrollment Specialist to orient potential participants and referral sources about the PACE program, collect intake information, set up assessments, and document care needs for new enrollments at the San Jose center.

You will interview families, provide tours, collaborate with the Interdisciplinary Team, and build relationships with community partners to support enrollment goals in a demanding healthcare setting.

Qualifications

• At least 1 year of experience in sales or community outreach.
• Current BLS and First Aid certification.
• Minimum 1 year experience working with frail/older adults.
• Experience giving community presentations.
• Fluency in English; Cantonese/Mandarin required; other languages a plus.

Responsibilities

• Serve as first contact and liaison between PACE team and potential participants and referral sources.
• Provide clear information about PACE and enroll eligible participants through screening and intake.
• Gather demographic information and required paperwork; schedule assessments; document care needs.
• Give tours of the PACE Center to participants and families; present enrolling cases to the Interdisciplinary Team.
• Develop and maintain relationships with referral sources and community organizations.
• Coordinate outreach education and targeted marketing with referral networks.
