The position requires RN licensure in Connecticut, 2–3 years of SNF/LTC experience, and prior overnight leadership. Proficiency with PointClickCare and strong independent judgment are essential.

• Experience coordinating admissions/transfers and night handovers.
• Strong clinical judgment and autonomous decision-making for night shift operations.
• Proficiency with EMR documentation and incident reporting.

Aufgaben

• Oversee overnight clinical operations 11:00 PM–7:00 AM across units.
• Act as primary night shift clinical resource for triage and emergencies.
• Conduct nocturnal rounds to monitor care, infection control, meds, and safety.
• Manage admissions/ transfers and morning shift handoffs to day leadership.
• Lead and support overnight RNs, LPNs, and CNAs.
• Ensure EMR documentation complies with DPH and CMS rules.
