Job summary

Behavioral Health Network, Inc. (BHN) is seeking an Outreach Counselor to join the Community Service Program (CSP) team in Western Massachusetts.

You will build therapeutic rapport with individuals served and help connect them to community resources and supports. You will collaborate with care team members, support discharge planning, and provide care management via in-person, Telehealth, and Telehealth modalities to promote health and community tenure.

Qualifications

  • 1–2 years in behavioral health, substance use, correctional, or human services field required.
  • Bilingual Spanish and English preferred.
  • HS diploma or GED required; CHW certs or higher preferred.

Responsibilities

  • Attend required meetings and trainings; maintain productivity and develop rapport with individuals served.
  • Maintain professional relationships with providers and supports involved in care.
  • Help individuals access community resources (health insurance, housing, nutrition, education, financial aid).
  • Coordinate with care team members with written consent.
  • Complete assessments, individualized and safety plans with Clinical Supervisor guidance.
  • Be primary contact for caseload and support discharge planning and re-engagement with outpatient services.

What You Will Do
  • Attend required meetings and trainings, maintain program productivity expectations, develop therapeutic rapport with individuals served, and complete other responsibilities as assigned.
  • Maintain professional and collaborative relationships with providers and supports involved in the care of individuals served.
  • Help individuals access community resources, including health insurance, food and nutritional assistance, housing placement and stabilization, financial and utility assistance, educational and vocational supports, and legal assistance.
  • Coordinate with appropriate members of the individual’s care team, including probation and parole officers, primary care physicians, nurses, mental health and substance use providers, and natural supports, with the individual’s written consent.
  • Complete Bio-Psycho-Social Assessments, Individualized Action Plans, Safety Plans, and Disenrollment Plans in collaboration with providers and supports, under the guidance of the Clinical Supervisor.
  • Act as the primary point of contact for individuals on the caseload, ensuring their needs are identified and addressed.
  • Meet individuals admitted to 24-hour facilities, including hospitals and substance use disorder treatment facilities, in person or via Telehealth and maintain regular contact during their stay to assist with discharge planning and re-engagement with outpatient providers, community services, and social supports.
  • Provide in-person, telephonic, and Telehealth care management supports to individuals served through an integrated health model that promotes improved health, independence, self-sufficiency, reduced risk of future criminal behavior, and increased community tenure.
  • Strong communication, organization, time management, cultural competence, collaboration, and relationship-building skills.
Who You Are
  • 1–2 years of work experience in a related behavioral health, substance use, correctional, and/or human services field required.
  • Bilingual Spanish and English speaking preferred.
  • Community Health Worker Certification and/or Bachelor’s Degree preferred.
  • High School Diploma or GED required.
  • Valid driver’s license required, with a vehicle available for use during all working hours; vehicle must be registered and inspected in accordance with 540 CMR 4.00.
  • Lived experience of involvement in the criminal justice system and recovery from behavioral health and substance use issues preferred.
  • Knowledge of community resources available to individuals served and how to access them.
  • Knowledge of mental health and substance use disorders, care management services, justice-involved populations and justice entities, and how to access community resources.
