Job summary

Behavioral Health Network (BHN) is seeking an Aftercare Coordinator to join our Community Support Services team. The role supports individuals served, ensuring referral to and engagement in the next level of care across Western Massachusetts, including Worcester.

Strong communication skills and a valid driver’s license are essential. We offer a patient-centered work environment with HEART values, ongoing training, and opportunities for professional growth within a longstanding statewide

Qualifications

  • High School Diploma or GED required.
  • Minimum of 1 year of relevant experience in behavioral health or community support services.
  • Familiarity with community referral sources preferred.
  • Valid driver’s license required.
  • Lived experience in recovery preferred.
  • Excellent verbal and written communication skills.
  • Familiarity with internet, word processing, and email systems.

Responsibilities

  • Provide information, referrals, and follow-up as specified in the treatment plan of individuals served.
  • Maintain working knowledge of levels of care criteria and resources available to individuals served.
  • Collaborate with treatment team and family to ensure referral to and engagement in next level of care.
  • Complete transition plans and required documentation in a timely and thorough manner.
  • Gather information and complete enrollments.
  • Provide direct and indirect services to support growth, independence, and safety.
  • Collaborate with internal program leadership and Access department to ensure appropriate referrals.
  • Collaborate with external agencies to identify best options for next steps in continuum of care.
  • Facilitate diverse treatment groups as needed.
  • Recognize and report concerns; contact supervisor in emergencies.
  • Transport individuals served as needed.
  • Perform other tasks as requested by the supervisor.

What You Will Do
  • Provide information, referrals, and follow-up as specified in the treatment plan of individuals served.
  • Maintain working knowledge of levels of care criteria, as well as internal and external resources available to individuals served.
  • Work with the treatment team and family of the individual served to ensure successful referral to and engagement in the next level of care.
  • Complete transition plans and required documentation in a timely and thorough manner.
  • Gather information and complete EIM/ESM enrollments.
  • Provide direct and indirect services to individuals served that support growth, promote independence, and ensure safety in all settings.
  • Collaborate with internal program leadership and the Access department to ensure appropriate referrals to the next level of care within the agency.
  • Collaborate with external agencies to identify the best options for individuals seeking the next step in their continuum of care.
  • Facilitate diverse treatment groups as needed.
  • Recognize and report specific concerns; understand when to contact a supervisor for guidance and/or in an emergency.
  • Transport individuals served as needed.
  • Perform other tasks as requested by the supervisor.
Who You Are
