PATH is seeking an Outreach Case Manager for the SSVF Team in Los Angeles, CA to help break the cycle of homelessness using a flexible, client-centered approach. You will conduct intake, develop ISP plans, provide referrals, and maintain HIPAA-compliant records in a fast-paced, mission-driven environment.

• Conduct intake and needs assessment to develop ISP addressing housing and income goals.
• Provide referrals and linkages to resources to meet ISP goals.
• Complete program assessments (e.g. VI-SPDAT) and document progress.
• Maintain HIPAA-compliant case files with timely notes.
• Ensure confidentiality and ethical boundaries in all interactions.
