Job summary

Qualifications

• Experience coordinating a multidisciplinary team in homeless services.
• Ability to develop treatment plans and document in HMIS or equivalent.
• Knowledge of HIPAA, CFR42 and reporting requirements.

Responsibilities

• Conduct immediate needs assessments and develop comprehensive treatment plans.
• Coordinate MDT collaboration and field work with homeless clients.
• Document notes and outcomes, maintain client files, and meet reporting deadlines.
• Provide case management, linkages to housing, health and support services.
