PATH in California is seeking a dedicated Case Manager to support individuals experiencing homelessness by guiding them through intake, ISP planning, referrals, and access to health and social services, with a focus on housing stability.

The role emphasizes trauma-informed care, rigorous documentation, and collaboration with partners to help clients secure permanent housing and achieve sustained wellbeing.

Responsibilities

• Conduct intake and develop ISP addressing housing goals and income; include medical, mental health, and social support needs.
• Provide referrals and connections to resources to meet ISP goals.
• Complete program-specific assessments (e.g., VI-SPDAT).
• Evaluate strengths and challenges; conduct 90-day reviews with participants.
• Maintain documentation per PATH policies; keep participant files complete and accurate.
• Maintain confidentiality per HIPAA and guidelines.
• Develop therapeutic relationships to support housing goals and health outcomes.
• Provide crisis intervention and risk assessment to enhance safety and self-management.
• Use motivational interviewing and trauma-informed care in service delivery.
• Maintain up-to-date knowledge of community resources to deliver wrap-around services.
