Colorado Coalition for the Homeless seeks a Case Manager I to collaboratively develop and implement care plans, coordinate services, and monitor progress for clients experiencing homelessness or transitioning out of homelessness.

The role emphasizes motivational interviewing, strengths-based approaches, and trauma-informed care, with outreach, referrals, and crisis intervention as core duties. Strong documentation and coordination with health and housing services are essential.

Competenze

• High School diploma or GED required.
• Bachelor’s degree in Psychology, Social Work, Human Services, Criminal Justice preferred.
• Basic Life Support (BLS) certification required within 30 days of employment.
• Must have and maintain a Colorado driver’s license within 30 days of hire.

• Manage a caseload of clients with high service utilization and complex health needs.
• Maintain collaborative relationships with community partners and health providers.
• Work with a team under supervision and support problem-solving efforts.
• Provide wrap-around care management, crisis support, and engagement.
