Job summary

Colorado Coalition for the Homeless is seeking a Case Manager I in Denver to deliver outreach and wrap-around care to residents experiencing homelessness or at risk. You will coordinate services, advocate for clients, and document progress in the organization’s systems.

The role emphasizes motivational interviewing, trauma-informed care, and collaboration with health teams and community partners to promote long-term housing solutions.

Qualifications

• High School Diploma or GED required.
• Bachelor’s degree in Psychology, Social Work, Human Services, Criminal Justice preferred.
• Basic Life Support Certification required within 30 days of employment.
• Must have and maintain Colorado driver’s license within 30 days of hire.

Responsibilities

• Manage a caseload of clients with high service utilization and disabilities.
• Maintain positive and cooperative relationships with community partners and health providers.
• Work under direct supervision within department parameters as part of a team.
• Provide wrap-around care management to ensure consistent communication and engagement.
• Liaise between program management, client referral, and community partners.
• Respond to crises with de-escalation and appropriate referrals.
• Develop crisis and safety plans tailored to client needs.
• Track client data, engagement, and outcomes; maintain up-to-date charts.
