Job summary

Colonial Group is seeking a Case Manager I to collaborate with an integrated health team and community partners. You will develop care plans, coordinate services, monitor progress, and advocate for clients experiencing or at risk of homelessness.

Responsibilities include crisis intervention, referrals, and data/documentation maintenance, with a focus on trauma-informed, strengths-based approaches and housing support services.

Qualifications

• High School Diploma or GED required.
• Bachelor’s degree in Psychology, Social Work, Human Services, or Criminal Justice preferred.
• Basic Life Support Certification required within 30 days of employment.
• Colorado driver’s license required within 30 days of hire.

Responsibilities

• Manages a caseload of clients with high-service utilization and disabilities.
• Coordinate services and resources with community partners and health providers.
• Develop and implement wrap-around care plans and crisis interventions.
• Maintain documentation and data in the EHR system.
• Advocate for clients and facilitate referrals to external services.
• Transport clients as needed and engage in outreach to assess basic needs.
