The Colorado Coalition for the Homeless is seeking a Case Manager I to coordinate care plans, connect clients to housing, health care and support services, and document progress in our EHR system.

This role involves delivering outreach in the Denver metro area from 2100 Broadway, requiring a valid Colorado driver’s license and travel to central Denver region as needed. Join a mission-driven team dedicated to ending homelessness.

Qualifications

• 1–2 years of experience working with a diverse population who are experiencing mental health challenges, preferred.
• Colorado driver’s license within 30 days of hire required.
• Bachelor's degree in relevant fields preferred; combination of education and experience considered.
• Must have and maintain in good standing a Colorado driver’s license within 30 days of hire.

Responsabilités

• Manages a caseload of clients with high-service utilization and disabilities including substance use and significant primary and behavioral health care needs.
• Maintain positive and co‑working relationships with community partners and health providers.
• Works under direct supervision and supports team problem-solving.
• Support clients with wrap-around care management and crisis engagement.
• Facilitate referrals to external services and coordinate care with other providers.
