Job summary

Pittsburgh Mercy in Pittsburgh is seeking a PATH Clinical Case Manager to provide outreach and support to individuals experiencing homelessness across the community. You will help clients access housing, behavioral health services, substance use treatment, and public benefits; conduct outreach in shelters, encampments, and community sites.

Responsibilities include documenting in HMIS and Avatar, balancing priorities, and complying with laws and Pittsburgh Mercy's policies to deliver

Qualifications

Master's/Bachelor's/Associate/HS with related social service experience as listed.
Valid driver's license required.
Ability to operate organizational vehicle/van.
Act 33/34/73 clearances and pre-employment drug test.

Responsibilities
