Job summary

Pittsburgh Mercy is seeking a PATH Clinical Case Manager to provide outreach and support to individuals experiencing homelessness across Allegheny County. The role connects clients with housing, behavioral health services, substance use treatment, and public benefits, across shelters and community sites.

Qualifications

• Master's/Bachelor's/Associate/H.S. diploma in social services with related experience per criteria.
• Valid driver's license is required.
• Ability to operate organizational vehicle/van.
• Act 33/34/73 clearances required.
• Pre-employment drug test required.

Responsibilities

• Provide direct outreach and support to individuals experiencing homelessness.
• Help clients access Continuum of Care housing, behavioral health services, and public benefits.
• Conduct outreach in shelters, encampments, community sites, and agency locations.
• Document accurately in HMIS and Avatar systems and manage multiple priorities.
